Rather than being an afterthought, contextual analysis enables the meaning and inner workings of our main variable of interest to be better illuminated (Collins et al., 1999). Context, according to the Oxford dictionary, is defined as “the circumstances that form the setting for an event, statement, or idea, and in terms of which it can be fully understood and assessed” (OED, 2014). To further such understanding, in this article we explore how context is understood in health systems and policy research (HPSR) and from that basis present findings from our literature review on HC contextual factors.Įmphasis on understanding context in HPSR (Walt and Gilson, 1994) is part of what distinguishes it as a social science subject (Sheikh et al., 2011). One part of unlocking their potential to engage communities and improve health care quality and coverage lies in better understanding their contextual location within health systems and societies. Despite their widespread prevalence, their contributions vary due to their diversity in formation, roles, resources and mandates. In many countries, they are a familiar fixture of health systems, and can be effective in improving quality and coverage of health care, as well as improving health (McCoy et al., 2012). Health committees (HCs) are one of the better documented mechanisms in an incipient empirical evidence base on community accountability in health systems (Molyneux et al., 2012). Such an understanding of context points to its contingent and malleable nature, links it to theorizing in HPSR, and clarifies areas for investigation and action. While reforms focus on tangible HC inputs and outputs (training, guidelines, number of meetings held), our review of contextual factors highlights the dynamic relationships and broader structural elements that facilitate and/or hinder the role of health committees in health systems. In addition, our review found that contextual elements are dynamic and porous in nature, influencing HCs but also being influenced by them due to the permeability of HCs. By doing so, our review distinguishes between contextual elements that are relatively well understood and those that are not. While many attempts at describing context in HPSR result in empty arenas, generic lists or amorphous detail, we suggest anchoring an understanding of context to a conceptual framework specific to the phenomena of interest. Through an iterative process of exploring previous attempts at understanding context in health policy and systems research (HPSR) and the HC literature, we developed a conceptual framework that delineates these contextual factors into four overlapping spheres (community, health facilities, health administration, society) with cross-cutting issues (awareness, trust, benefits, resources, legal mandates, capacity-building, the role of political parties, non-governmental organizations, markets, media, social movements and inequalities). After screening with inclusion and exclusion criteria, we focused on 44 articles. We found 390 English language articles from journals and grey literature since 1996 on health committees, councils and boards. We undertook a narrative review to better understand the contextual features relevant to HCs, drawing from Scopus and the internet. Despite their widespread prevalence, HC functions vary due to their diversity and complexity, not least because of their context specific nature. Health committees, councils or boards (HCs) mediate between communities and health services in many health systems. In both groups, we only included patients who had received intravenous vasopressors for at least 12 h after surgery (see Additional file 1: Figure S1). The control group consisted of patients from two observational prospective studies that were done between 20 in the same institution, that included adult patients undergoing cardiac surgery with CPB. In our institution, there is no protocol to guide the use of midodrine and its prescription is at the discretion of the ICU physician. The midodrine group included all consecutive patients having received midodrine within the first week (168 h) after cardiac surgery with CPB, between January 2014 and January 2018 at the 24-bed surgical ICU of the Montreal Heart Institute. Our hypothesis was that midodrine would safely accelerate ICU discharge and be associated with more days free from ICU at 30 days. Other objectives consisted of describing the clinical contexts in which this strategy was used as well as the observed prescription patterns. The main objective of this study was to explore relevant clinical impacts and assess the safety profile of this strategy. In an attempt to hasten weaning of intravenous vasopressors and accelerate ICU discharge after cardiac surgery, intensivists in our center have been increasingly using midodrine in patients with post-CPB vasodilation. The use of midodrine in the context of post-CPB vasodilation represents an interesting approach, given the somewhat predictable temporal pattern of hypotension, relatively short length of stay and availability of hemodynamic monitoring. Most patients in these cohorts presented with hypotension from sepsis and none had undergone cardiac surgery with CPB. Results from small cohort studies suggest that weaning of intravenous vasopressors is accelerated with midodrine administration, without significant side effect, although a recent meta-analysis has not confirmed such results. Still, its characteristics make midodrine an attractive agent for ICU patients presenting prolonged hypotension attributed to vasodilation who are otherwise stable, and this particular use has been increasingly described in the last few years. Midodrine is an orally administered alpha agonist with predictable hemodynamic effects and an established safety profile outside of the ICU, although its safety for patients with intradialytic hypotension has been questioned. While strategies have emerged to prevent and address post-CPB vasodilation, including refinement of surgical techniques as well as the development of more biocompatible CPB tubing, there is currently no specific treatment postoperatively which would reduce dependence on intravenous vasopressors and length of stay in the ICU. These patients, besides receiving more intravenous fluid and vasopressors after their surgery, stay longer in the intensive care unit (ICU) and are at increased risk of kidney injury and mortality. This phenomenon is most probably multifactorial, with inflammatory and ischemia–reperfusion insults at the forefront. There was no difference in length of intravenous vasopressors (63 vs 44 hours, p = 0.052), rate of ICU readmission (6 (8.1%) vs 2 (2.7%), p = 0.092) and occurrence of severe kidney injury (11 (14.9%) vs 10 (13.5%) patients, p = 0.462) between groups.ĭepending on the definition used, between 5 and 25% of patients undergoing cardiac surgery with cardiopulmonary bypass (CPB) will present sustained hypotension after surgery presumably attributed to loss of peripheral vascular resistance.
